Outreach Visits
The Outreach MCH Program delivers Key Age and Stage assessments in childcare centres, kindergartens, and early learning settings, providing an easy and familiar way for families to engage.
An Outreach MCH nurse will be visiting your child’s childcare facility particularly to see children who are:
- Overdue for a MCH assessment.
- Between 3.5 years and 4 years of age and
- not yet had their 3.5 year MCH assessment.
- has never had a MCH assessment.
- Where you or the childcare staff have concerns about your child’s development or growth.
If you would like your child to be seen by our Outreach MCH nurse, please complete the details and consent below.
Families who prefer to attend an appointment at one of our seven MCH centres across Glen Eira can call 9524 3333 to book.
About this assessment

Childcare centre staff will be present during the assessment. There is no need for your child to have clothes removed for this assessment, except perhaps shoes when height measuring occurs.
You will receive feedback about this assessment afterwards using the contact details you provide below.
