Immunisation catch-up schedule request
Access to free vaccines through the National Immunisation Program (NIP) requires that an individual holds, or is eligible for, a Medicare card. You can check eligibility here: eligibility-criteria-for-free-vaccines
If you/your child does not have a Medicare card and believe you/they are eligible, please upload a statement explaining this in the form below.
If you/your child is not eligible your GP can review immunisation histories, update the Australian Immunisation Register (AIR), and administer any required catch up vaccinations.
